ΣΠΙΝΘΗΡΟΓΡΑΦΗΜΑ ΘΥΡΕΟΕΙΔΟΥΣ ΚΑΙ ΚΑΡΚΙΝΟΣ (Παλαιότερες απόψεις και νεώτερα δεδομένα)
Περίληψη
Σε 104 ασθενείς (16 άνδρες, 88 γυναίκες, ΜΗ=45,2 έτη) που χειρουργήθηκαν για μονήρη όζο του θυρεοειδούς (1986-87), η ιστολογική υφή της βλάβης (τελική διάγνωση) συσχετίσθηκε με την προεγχειρητική σπινθηρογραφική της απεικόνιση Τ^99 ). Από τους 73 σπινθηρογραφικά «ψυχρούς» όζους 10 (13,6%) ήταν κακοήθεις (4 θηλώδη, 2 θυλακιώδη, 3 μεικτά και 1 αδιαφοροποίητο καρκινώματα) και 63 (86,4%) καλοήθεις (32/73-44% θυλακιώδη αδενώματα, 29/73-40% οζώδεις υπερπλασίες και 2/73-2,4% θυρεοειδίτιδες). Από τους 31 σπινθηρογραφικά «θερμούς» όζους 5 (16%) ήταν κακοήθεις (θυλακιώδη καρκινώματα) και 26 (84%) καλοήθεις (19/31-61% θυλακιώδη αδενώματα και 7/31-23% θυρεοειδίτιδες). Αντίστροφα σαν «θερμοί» σπινθηρογραφικά εμφανίστηκαν τα 33,3% των καρκινωμάτων και τα 30% των καλοήθων όλων (διαφορά μη σημαντική στατιστικά ρ=0,74515). Όσον αφορά ειδικότερα τη συχνότητα εμφάνισης ως θερμών όζων των θυλακιωδών καρκινωμάτων σε σύγκριση με τα θυλακιώδη αδενώματα και τις οζώδεις υπερπλασίες αυτή ήταν στατιστικά μεγαλύτερη του αναμενόμενου (ρ=1,7531 x 10^-2). Συμπεραίνεται ότι, σε αντίθεση με τις μέχρι σήμερα απόψεις, η πιθανότητα ένας «θερμός» όζος να είναι καρκίνωμα είναι η ίδια ή και μεγαλύτερη από έναν «ψυχρό» και ότι τα καλοήθη αδενώματα έχουν περισσότερες πιθανότητες να είναι «ψυχρά» απ' ό,τι «θερμά» στο σπινθηρογράφημα.Αναφορές
