ΟΞΕΙΑ ΥΠΟΤΡΟΠΙΑΖΟΥΣΑ ΠΑΓΚΡΕΑΤΙΤΙΔΑ ΣΕ ΧΩΡΙΣΤΗ ΕΚΒΟΛΗ ΤΩΝ ΠΟΡΩΝ
Περίληψη
Άντρας ηλικίας 72 χρονών, πότης, με ελεύθερο αναμνηστικό, το τελευταίο οκτάμηνο παρουσίασε 4 τυπικές κρίσεις οξείας παγκρεατίτιδας. Κατά τις κρίσεις παρουσίασε διόγκωση του παγκρέατος και διάταση του μείζονα παγκρεατικού πόρου. Στα μεσοδιαστήματα δεν υπήρχαν κλινικά και εργαστηριακά ευρήματα. Δύο προσπάθειες ERCP απέτυχαν λόγω μεγάλης στένωσης της εκβολής των πόρων. Χειρουργικά διαπιστώθηκε χωριστή εκβολή χοληδόχου και παγκρεατικού πόρου στο 12/λο με στένωση της τελικής μοίρας τους. 'Εγινε διαδωδεκαδακτυλική σφιγκτηροτομή και σφιγκτηροπλαστική και στους δύο πόρους. Μετεγχειρητικά παρατηρήθηκε υπεραμυλασαιμία μόνο για 24 ώρες. Το μετεγχειρητικό αποτέλεσμα ήταν άριστο και ο ασθενής είναι ελεύθερος ενοχλημάτων μέχρι σήμερα.Αναφορές
