CHOLEDOCHODUDENOSTOMY IN CASES OF CARCINOMA OF THE HEAD OF THE PANCREAS ΟR THE AMPULLA OF VATER
Abstract
The cases considerEd were patients in a rather bad condition with advanced carcinoma of the head pancreas or the ampulla of Vater and were treated in our Clinic during the last decade. We had 20 cases of carcinoma of the head of the pancreas (females 9, males 11) and 3 cases of carcinoma of the ampulla of Vater (F2.m1) with metastases to the liner in 7 cases, to omentum and parietal peritoneum in 5 cases and extention by contiquity into more than one neighoborigh structures with metastases to regional lymphnodew in the rest. Ιn all these we cases we performed the following drainage procedures: Cholecystogastrostomy in 7 patients, cholecystostomy in 3 patients, cholecystoduodenostomy in 1 patient, cholecystojejunostomy in 2 patients, choledochojejunostomy Roux-en-y in 3 patients and choledochoaenostomy in 7 patients. Comparing the operations performed, their complications, early and late postoperative coarce of the patients and their survival rate it appears that cholodechodenostomy is a good palliative procedure for pancreatic and ampullary carcinoma in advanced stage, with operative mortality in a very lοw lever as well as a lοw rate of postoperative complications and satisfactory survival.References
