RATIONAL USE OF PROPHYLACTIC ANDIBIOTICS ΙΝ APPENDICECTOMY

Abstract

The postoperative infections and their sequences remain the most common reason of morbidity and mortality in a Surgical Clinic. Therefore the preoperative and postoperative use of antimicrobial drugs is necessary. Everthough the problem hasn't yet found its definite solution and it is still under discussion. With this paper we want to notice the possibility of not using antibiotics on septic operations like appendicectomy with the expetion of some cases. Ιn this way the adverse reactions of the drugs and the development of resistant hospital bacteria are avoided. From Jan 1986 to Dec 1988 275 patients were operated for acute appendicitis in the A' Surg. Clinic of Κavala General Hospital. Twenty patients were excepted. The rest 255 were devided by acudental choice to three equal in number groups. Group A: Without antimicrobial prophylaxis. Group Β: preoperative antimicrobial prophylaxis. Group C: postoperative antimicrobial prophylaxis for four days. From the study of postoperative complications-Fever, infection of surgical wound, pneumonitis-adverse drug reactions, the postoperative morbidity and mortality no serius diffences among the three groups of patients were found. Ιn conclusion, from the study of the above cases it was found that the use of antimicrobial drugs is not necessary in appendicectomy except in special cases.
